Add Custom Survey Questions

Learn how to add custom survey questions.

  1. Go to Content > Content Library

  2. Search for the course you want to add the policy to and click Configure on the card

  3. Go to the Surveys tab

  4. Find which survey you want to set up and select Configure

  5. Click Add New Section

  6. Enter a Section Name (required) and a Section Description (optional)

  7. To the learner, a section is simply a page of questions. This name and description will act as the page title and instructions for your learners.

  8. You are only allowed a single section for your custom questions.

  9. Click Add Section to save the section you created

  10. Click the Section to expand it and then click Add a Question to select the question type: multiple choice, text input, integer, or decimal

  11. Multiple Choice – lets learners select one or more answers from a set of options; can be single-select or multi-select

  12. Text Input – lets learners enter text to answer the survey question

  13. Integer – lets learners enter numbers to answer the survey question

  14. Decimal – lets learners enter numbers with decimal values to answer the survey question

  15. Enter your question text and check the checkbox to indicate whether or not the question is required for learners. If you select Required, learners will not be able to submit the survey unless they have answered the question.

  16. Click Add Question

  17. If you added a multiple choice question, you’ll need to add the answer choices next. Expand the question you just created to add the answer choices

  18. Enter the text for your answer choice and click Add Answer

  19. Continue adding questions and answer choices until you have created all of your desired survey questions

  20. Once you have finished adding your questions, click Preview to see how the questions would render for learners

    1. Using the Preview tab, you can review how your questions will look for your learners. However, Preview mode does not display the rest of the survey – only your custom section.
